Your California privacy rights

California residents may have the rights described below under the California Consumer Privacy Act, as amended (CCPA), for personal information within the CCPA's scope. Azyck may also voluntarily honor certain requests when the CCPA does not require it. Certain information may be exempt, including information governed by the Gramm-Leach-Bliley Act, the California Financial Information Privacy Act, or other insurance, financial, employment, health, or legal regimes.

Sales and sharing status

Azyck does not sell personal information for money. Azyck does not knowingly sell or share personal information of consumers under 16.

Submit a request

Use any method Azyck makes available for the applicable request:

For requests to opt out of sale or sharing, use Cookie Preferences, submit the form, or enable a recognized opt-out preference signal such as Global Privacy Control.

Verification and timing

Azyck will use information provided in a request only to process, verify, document, and respond to the request, or as otherwise permitted by law. Verification will be proportionate to the request's risk and the sensitivity of the information. Azyck will confirm receipt and respond within the time required by applicable law. If the CCPA applies, the operational target is confirmation within 10 business days and a substantive response within 45 calendar days, with a permitted extension and notice when necessary.

Global Privacy Control and cookie preferences

Azyck honors Global Privacy Control and other legally recognized opt-out preference signals. The Cookie Preferences control is available in the footer and offers equally clear Accept, Reject Non-Essential, and granular choices.

Notice at collection

We collect the information you enter (such as name, email, telephone number, organization, appointment details, and message) to respond, schedule services, evaluate requested insurance or risk services, maintain records, and protect our systems. Do not include passwords or sensitive incident evidence. See our Privacy Policy and this page for details, retention, disclosures, and rights.
