Producer Compensation Disclosure

Purpose
This disclosure explains how Azyck may be compensated for insurance products and services. "Producer" may include an insurance agent, broker, agency, managing general agent, or other licensed insurance intermediary, depending on the transaction and applicable law.
Types of compensation
Depending on the product, jurisdiction, and services provided, Azyck may receive one or more of the following, but only if applicable and legally permitted:
- Base commission: A percentage of premium or a fixed amount for placing, selling, or servicing a new or renewal policy.
- Broker or service fees: A separately disclosed fee paid by the customer for specific services, where permitted and agreed in writing. A fee may be in addition to commission.
- Contingent, supplemental, or incentive compensation: Compensation based on factors such as aggregate premium, production, growth, retention, profitability, quality, service, or other performance measures, generally measured across a book of business rather than a single customer.
- Administrative, program, technology, underwriting, risk-management, marketing, referral, data, or other service compensation: Paid under an agreement for services performed.
- Compensation from insurance-market participants: Paid directly or indirectly by an issuing insurer, a DOXA-affiliated entity, or another insurance-market participant, to the extent permitted by law and the applicable agreements.
- Non-cash benefits: Training, conferences, marketing support, technology, awards, meals, or other lawful business benefits of reasonable value.
Factors affecting compensation
Compensation may vary by insurer, product, policy type, premium, new or renewal status, risk classification, services provided, volume, performance, and other lawful factors. Multiple producers, wholesale brokers, managing general agents, administrators, associations, referral sources, or other parties may receive compensation in the same transaction.
Customer-paid fees and premium
Any customer-paid producer or service fee will be disclosed as required and, where required, documented in a separate written agreement. A producer fee is not an insurance premium unless the governing documents and law state otherwise. Premium, taxes, assessments, carrier fees, financing charges, and producer fees should be itemized where required.
Our commitment and conflicts
Compensation can create an incentive to place business with a particular market or product. Azyck seeks to consider customer needs, requested coverage, underwriting eligibility, product availability, terms, service, and price, consistent with its legal duties and market access. The markets presented may not include all available insurers or products.
